Program Description

Patient Navigation Level 2 builds upon the foundational knowledge from Level 1, offering advanced training to address the complexities of navigating today’s healthcare systems. This course focuses on managing multifaceted patient cases, equipping participants with the skills to advocate for patients facing significant barriers to care. Participants will learn to work effectively in interdisciplinary teams, developing tailored strategies that promote equitable access to healthcare while addressing the unique needs of diverse populations.

With an emphasis on systemic advocacy, Level 2 empowers participants to drive improvements within healthcare organizations and systems. The course delves into strategies for overcoming fragmented care, improving care transitions, and ensuring patient-centered outcomes. By the end of the program, participants will have the tools and confidence to take on advanced navigation roles, foster collaboration across disciplines, and make meaningful contributions to creating a more inclusive and accessible healthcare system.

Learning Outcomes

By the end of the course, participants will be able to: 1.Solve complex patient navigation challenges using advanced techniques. 2.Navigate healthcare systems to address gaps and improve patient outcomes. 3.Advocate effectively for equitable access to care for vulnerable populations. 4.Strengthen communication and coordination among care teams. 5.Implement culturally informed strategies to support diverse patients. 6.Evaluate navigation effectiveness and propose improvements. This advanced program provides an interactive and comprehensive approach to mastering the challenges of patient navigation in modern healthcare settings.

3 hrs/day over 4 days online

Fully Online - Live Instructor Led sessions

In addition to class time, 1.5 hours of self study time per course day

Requirements for Enrolment

This course is ideal for:
• Healthcare professionals, patient navigators, and allied health staff who have successfully completed Patient Navigation Level 1.
• Those seeking to enhance their ability to manage complex healthcare cases and drive improvements in patient care.

This program is offered in English. It is your responsibility to ensure you meet the language requirements. You may be asked to demonstrate language proficiency.
